RCBA and TAPPS requires athletic forms for both athletes and coaches. Review the following rules to determine which forms need to be submitted to the school office. There may also be additional forms required that are specific to a team sport. Please check for attachments on the team sports page and/or contact the coach to ensure that you have completed all of the required forms. You can download or view the general athletic forms under the Attachments section below.

RCBA Student Athletes

View the Student Fan Behavior Presentation.
Turn in a Sports Physical Form if you are a new student athlete.
Turn in a Sports Physical Form if you are in the 7th grade or are in any high school grade.
Turn in a Student Acknowledgment of Rules Form if you are a high school athlete
Turn in a Transfer Form that is signed by your former school administrators if you are a transfer student who participated in TAPPS athletics during the previous season.

RCBA Coaches

All RCBA coaches must complete the following requirements prior to their season:

View the Solicitation of Students Presentation.
View the Student Eligibility Presentation.
View the Officials Presentation.
View the All District / All State Presentation.
View the Parent Fan Behavior Presentation.

Once you have completed viewing the videos and presentations

Complete the Online SCOPE Certification.
Notify the school office when you have completed the requirements.
